To download the forms:
   1. Left-click once on the document links. This will open the File Download dialog box. Select the Open or Save button.
or
   2. Right-click the document link. If you are using an Internet Explorer browser select the Save Target As, in the Netscape browser select Save Link Target As. Save the file to a directory on your computer.

The documents are available in two formats, a Microsoft Word document file and a rich text format.
